Nutrition Consultations 

Initial: $150.00

This dietary consultation involves a complete nutritional assessment. Our objective with this consultation is to assist us in understanding your needs, as well as formulating a practical plan on how to achieve your nutritional goals. 

Extended Review: $110.00

Standard Review: $90.00

The review nutrition consultations focus on providing you with the tools needed to achieve your specific health goals. These consultations are recommended to be booked in on an as need basiss. 

Couple Consultation: $235.00 ($155.00 review)

We ask that you allow 90 minutes for an initial and 45 minutes for a review couples nutrition consultation. 

 

Specialsied Eating Disorders Consultation Fees  

We provide specialist treatment for anorexia nervosa, bulimia nervosa, binge eating disorder, avoidant- restrictive food intake disorder, other atypical disorders, weight management and obesity. 

Fees include coordination and communication with your GP and any other health care providers where necessary. 

Initial: $195.00 

Review: $125.00 - 45 min extended review, $105.00 - 30 min standard review

Any diagnosed eating disorder patients will be entitled to access 20 rebated sessions per calendar year through Medicare. A GP referral is required to access this rebate of $58.30 per session. Please note Hardy Nutrition do not offer bulk billing for eating disorder consults.

Medicare

If you have a chronic disease and have a management plan with your GP you could be eligible for five allied health appointments each calendar year. Ask your GP for a CDMP or TCA referral to see a dietitian at Hardy Nutrition Dietary Consulting. 

Medicare rebates of $58.30 can be claimed on the day of the dietary appointment. 

Please note we are a private billing clinic. 

If you are seeking our services under a chronic disease care plan referral from your GP these will only be bulk billed for standard consults if you hold a health care concession card/pension card, otherwise gap fees are required if you do not hold a health care concession card or for extended more complex appointments such as coeliac disease, food intolerance and eating disorders (our fees are available through our website http://hardynutrition.com.au/fees-perth-dietitian.html). We do not bulk bill for eating disorders. 

If you do not hold any of the above concession cards, you will be asked to pay a gap on the day of your consult. A gap fee will be charged as follows:

Chronic Disease Management Plan standard consult: $31.70 payable on the day 

Chronic Disease Management Plan extended consult: $91.70 payable on the day 

Eating Disorder Chronic Disease Management Plan gap fee dependent on length of consult: $46.70 - $136.70 payable on the day. 

Note we do not bulk bill for concession card holders on a Saturday. Gap fees will apply for any Saturday appointments.

 

 

Private Health Insurance

Costs may be claimed for dietetic services with private health insurance 'extras' cover. Please check with your health insurance company to see if you're covered. When contacting your health insurance provider please quote item numbers 500 for an initial consult or 600 for a review consult. They will then be able to provide you details on your entitled rebate. 

 

Veterans Affairs
We are a registered DVA provider and offer DVA-funded nutrition consultations to applicable card holders with a GP referral. Please check with your GP if you think you may be eligible. 

Once your doctor has made a referral or if you simply want to book an appointment with a Dietitian without a referral contact us to book your appointment. If you have further questions about our nutrition services or fees feel free to call us on 9494 3790. 

Please inform us by telephone at least 24 hours' before you appointment if you are unable to attend. Along with providing the opportunity to reschedule, this respects your Dietitians time and allows for other clients to fill your appointment. 

If you fail to cancel or reschedule your appointment with a minimum of 24 hours' notice you will be charged the full fee for the session (no rebate or refund is available for this charge).  If wishing to reschedule a failed to attend appoinment you will be required to pay appointmnet fees in full, upon booking your appointment. You may then be entitled to claim any rebates through your health insurance provider or medicare after attending your appointment. 

We never double book appointments. Therefore please respect that 24 hours' notice is required for all cancellations or rescheduling of appointments.

Financial Policy 

It is the patients resonsibiliy to ensure that your fees are paid in full. Hardy Nutrition Dietary Consulting will verify your coverage and bill Medicare on your behalf. However, the patient is ultimately responsible for payment of their bill. If your Medicare claim is declined for any reason, it is the responsibility of the patient to pay any outstanding fees.
 
The patient is responsible to pay any outstanding deductible and co-payments as determined by Medicare under your chronic diasease care plan.
 
For private consults, all fees are due in full at the time of service. The patient is responsible to pay the full balance on the day of services. We can process health insurance claims in clinic with HICAPS with a valid health insurance card.
 

 

Referral Policy: 

 

In order to claim any rebates through Medicare under a chronic disease care plan, your referral must be received and presented in clinic prior to your consult. 

 

If we do not have evidence of a valid chronic disease care plan referral at the time of your consult, we are unable to claim your rebates from Medicare on your behalf, you will be asked to pay your fees in full. You may then be able to claim any rebates entitled to you direct from Medicare.
